  • Rides

    I-RIDE takes seniors to medical appointments, dialysis, and errands.

  • Meals

    Home-delivered meals reach seniors who cannot easily cook or shop, and the Senior Farmers Market Nutrition Program helps pay for fresh produce.

  • Care coordination

    Care Transitions pairs a coach with seniors 60 and older who are returning home after a hospital stay, to help them recover safely.

  • Medicare and benefits counseling

    Free, unbiased counseling helps compare Medicare plans, and Prescription Drug Assistance helps with the cost of medication.

  • Staying safe and well

    Senior Cool Care helps seniors stay safe when the weather turns hot. If you suspect abuse, neglect, or exploitation, a 24-hour hotline is available to report it.
